Step 1: Book Online – No Referral Needed

You can book directly through our website at adhdab.ca. There’s no referral required, and no waitlists.

After booking, you’ll receive a confirmation email with all the details you need to prepare for your assessment, including how to access your secure video appointment, and what forms need to be completed beforehand.


Step 2: Complete Your Intake Forms

Before your appointment, you’ll be asked to fill out:

  • A detailed intake questionnaire about your symptoms and background

  • Validated ADHD screening tools (such as the ASRS and WFIRS-S)

  • Optional: input from a partner, family member, or friend (not required, but can be helpful)

These forms give your provider a comprehensive view of your current challenges and life context. We ask that these forms are completed no later than 48 hours before your appointment. 


Step 3: Meet with Your Clinician (Telehealth Appointment)

Your assessment will be conducted virtually via secure video call with one of our licensed nurse practitioners, who are trained in diagnosing and treating adult ADHD.

During this 60 minute session, we’ll explore:

  • Your current symptoms and how they impact your daily life

  • Your childhood and educational history

  • Work, relationships, and patterns of behavior

  • Any co-occurring concerns (e.g. anxiety, depression, burnout)

We’ll always take the time to explain what we’re seeing and answer your questions—no jargon, no judgment.


Step 4: Receive a Diagnosis (If Criteria Are Met)

If your symptoms meet the criteria for ADHD, your clinician will explain the diagnosis in clear terms. We’ll walk you through:

  • What your specific presentation of ADHD looks like

  • What that means for you now—and going forward

  • How your strengths and struggles fit the picture

If ADHD doesn’t seem like the best fit, we’ll offer alternative suggestions and recommendations to help you move forward.


Step 5: Personalized Treatment Plan

We believe in collaborative care. If you're diagnosed with ADHD, we’ll work together to create a treatment plan that fits your lifestyle and goals.

Treatment options may include:

  • Medication management (offered directly through our clinic)

  • Coaching or therapy referrals

  • Executive functioning strategies

  • Psychoeducation and long-term support

Our goal is to help you function better, not just get a label.

You will receive a detailed assessment report that can be used as medical proof of your diagnosis for a wide range of services and programs. This report can also be used as a guide to help your primary care provider start treatment. 


Step 6: Follow-Up & Support

You’re not on your own after the assessment. We offer:

  • Check-ins to adjust your plan

  • ADHD-friendly care that respects your time, energy, and brain

  • ADHD-centred counselling services 

We also provide documentation for school, work, or insurance if needed.
